Getting Mounjaro dosing right is crucial for both results and tolerability. Rush the process and you'll feel terrible. Go too slowly and you'll get frustrated with slow progress. Here's the recommended schedule and what to expect at each stage.

The Standard Titration Schedule

Mounjaro comes in six dose strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg, and 15mg. Everyone starts at 2.5mg regardless of body weight. This isn't the treatment dose; it's the adjustment period.

Standard Timeline:

  • Weeks 1-4: 2.5mg once weekly
  • Weeks 5-8: 5mg once weekly
  • Weeks 9-12: 7.5mg once weekly
  • Weeks 13-16: 10mg once weekly
  • Weeks 17-20: 12.5mg once weekly
  • Week 21+: 15mg once weekly (maximum dose)

Each dose level lasts a minimum of four weeks. This gives your body time to adjust before adding more medication. Some people stay at lower doses if they're achieving their goals with good tolerability.

What Happens at Each Dose

2.5mg (Weeks 1-4)

The starter dose. You might notice some appetite reduction, but many people feel almost nothing. Weight loss is minimal, usually 0.5-1.5kg in the first month. Don't be discouraged. This phase is about letting your gut adapt to the medication.

Some people experience mild nausea, especially after large meals. If you do, eat smaller portions and avoid greasy or heavy foods. This is your body signaling that it's responding to the medication.

5mg (Weeks 5-8)

The first therapeutic dose. Most people start noticing real appetite suppression here. Food becomes less interesting. You might catch yourself forgetting to eat lunch or feeling full after half your usual portion.

Weight loss picks up, typically 1.5-2.5kg during this month. Nausea may increase briefly when you first step up, then settle within a week or two. Stay hydrated and prioritize protein at meals.

7.5mg (Weeks 9-12)

Effects strengthen. Appetite is notably reduced for most patients. You might find certain foods, especially sweets and fried items, genuinely unappealing rather than just resistible.

This is often when people in clinical studies started seeing accelerated weight loss. If you're losing steadily and tolerating the medication well, your provider might keep you here rather than pushing higher.

10mg (Weeks 13-16)

Many patients find their maintenance dose here. The 10mg strength provides strong appetite control for most body types. In clinical trials, 10mg produced about 20% average weight loss, only slightly less than the maximum 15mg dose.

Side effects at 10mg are more pronounced than lower doses. Constipation becomes more common. Some people experience sulfur burps or increased acid reflux. These can usually be managed with dietary adjustments.

12.5mg and 15mg (Week 17+)

Maximum doses. Reserved for patients who need additional appetite suppression or who haven't reached their goals at lower doses. The most significant weight loss in clinical studies was achieved at the 15mg dose.

Not everyone needs or tolerates these higher doses. If 10mg is working well, there's no requirement to increase. Higher isn't automatically better if the medication is already doing its job.

Pick a day of the week that works consistently for you. Many people choose weekends so they can manage any side effects at home. Others prefer mid-week to avoid weekend social eating when appetite is most suppressed.

You can inject any time of day. Morning, afternoon, or evening all work. The key is consistency. If you inject Sunday mornings, keep injecting Sunday mornings.

If you miss a dose by a day or two, take it when you remember and resume your normal schedule. If you're more than four days late, skip that dose and wait for your next scheduled injection day.

Practical Tips for Nigerian Patients

Plan your injections around your refrigeration situation. If you have reliable power, keep your medication properly stored. If NEPA is inconsistent, consider injecting shortly after receiving a fresh supply rather than storing for weeks.

Stay well hydrated, especially in hot weather. Dehydration worsens constipation and nausea, two common side effects. Aim for at least 2-3 liters of water daily, more if you're sweating heavily.

Schedule your dose increases after consulting with your provider. They can help you troubleshoot any issues and ensure you're progressing safely. Self-adjusting doses without guidance can lead to problems.
